Two: Send the name and location of your company and the expected date of performance to turnstilepermission AT johndishwasher DOT com. (Yes, .com not .org). For example, "We are a street theater troupe known as the Screaming Javelinas in Tucson, Arizona and we intend to perform this work in May of 2010." That's good enough.


Three: Mention "johndishwasher.org" somewhere in your program (if you have one).
